Medical Baseline Program overview


The Medical Baseline Program, also known as Medical Baseline Allowance, is an assistance program for residential customers who depend on power for certain medical needs. Learn more in a short video.

PCIA Phase-Out (Existing Medical Baseline Customers)

The program offers two kinds of help

To qualify for Medical Baseline Program, a full-time resident in your home must have a qualifying medical condition and/or require use of a qualifying medical device to treat ongoing medical conditions. Only one Medical Baseline application per household is required.

Qualifying medical conditions

Some examples of qualifying medical conditions include:


  • Paraplegic, hemiplegic or quadriplegic condition
  • Multiple sclerosis with additional heating and/or cooling needs
  • Scleroderma with additional heating needs
  • Life-threatening illness or compromised immune system, and additional heating and/or cooling are needed to sustain life or prevent medical deterioration
  • Asthma and/or sleep apnea

Qualifying medical devices

Some examples of qualifying medical devices include:


  • Motorized wheelchair/scooter
  • IPPB or CPAP machines
  • Respirator (all types)
  • Hemodialysis machine
  • Iron lung

Apply or recertify online


  1. Fill out and submit the online application/recertification form.
    NOTE: Under the form's Terms & Conditions, if "I Acknowledge" does not activate by turning yellow, try this on your computer:
    • Go to Zoom (It's usually in browser settings in the upper right corner of your screen).
    • Click "+"; to enlarge text and reopen the Terms & Conditions window.
  2. Upon form completion, you will receive an email with a confirmation number and instructions for your medical practitioner.
  3. Share the instructions and your confirmation number in the email you received with your medical practitioner.
  4. After your medical practitioner fills out their part of the form and confirm your eligibility you will be enrolled in the Medical Baseline Program.

IMPORTANT: Once you submit an online application/recertification form, your medical practitioner also needs to confirm your eligibility online. Also, we will not be able to process paper applications when there is an online application pending.

  1. Download and print application/recertification form (PDF, 171 KB)
  2. Complete Part A of the application/recertification form and sign
  3. Have your medical practitioner complete Part B of the application/recertification form and sign
  4. Mail your completed form to:

Customers who do not have a permanent qualifying medical condition will complete a Self-Certification after the first year and will be required to recertify with a qualified medical practitioner's signature for the following year.


You can request an application/recertification form and obtain more information about the Medical Baseline Program by contacting PG&E at 1-800-743-5000.


You can also request Medical Baseline Forms in alternate formats, such as large print, Braille or audio., Email your request to: CIACMC@pge.com. Include your name, mailing address and phone number. Please allow 5-7 business days for processing. Or, call 1-800-743-5000 to request an alternate format.


Deaf/Hard of Hearing customers using TTY (a specialized device for the Deaf and hard of hearing) may call California Relay Service at 7-1-1.

Customers certified by their qualified medical practitioner as having a permanent qualifying medical condition must self-certify their eligibility every two years. This is to confirm their continued residence at their service address and does not require a qualified medical practitioner’s signature.


Customers who do not have a permanent qualifying medical condition will complete a Self-Certification after the first year and will be required to recertify with a medical practitioner's signature for the following year.


Note: This cycle of self-certification followed by recertification repeats for as long as the customer is enrolled in the program.


If you are an active Medical Baseline customer with a non-permanent medical condition and need to recertify for continued eligibility, go to the Apply/Recertify section of this page and follow the steps to complete the application.


Active PG&E Medical Baseline customers who have received a notification to self-certify may self-certify online and receive an immediate confirmation of renewal:


  1. Sign in to renew coverage with your PG&E online account information. If you don’t have a PG&E online account, click on "One-Time Access."
  2. Complete the form and submit.

Or, submit a paper self-certification form by mail:


  1. The Medical Baseline Self-Certification form is included with your renewal notification letter. You may also print a copy of the self-certification form (PDF, 830 KB).
  2. Complete form and sign.
  3. Mail your completed form to:
