For gas day February 21, 2003, Pacific Gas and Electric Company's California Gas Transmission called a System-Wide OFO even though the requirements for a Customer-Specific OFO were met.

On the morning of February 20, 2003, CGT experienced difficulties with an internal report. During this short time, CGT could not accurately determine which customers would be targeted by the CS-OFO. Rather than delay the announcement of the OFO, CGT chose to call a SW-OFO.
