Medical Baseline/Life Support - Un-enroll

Use this form to un-enroll from PG&E's Medical Baseline/Life Support programs. Important note: by un-enrolling, you will no longer receive Medical Baseline quantities. If Life Support is listed on your account, the Life Support status will also be removed.

You may have received an insert letter from PG&E for Notification Preferences with a reference to un-enroll. If you have that insert include the 8-digit ID code starting with "MB" from the the lower-left corner of the insert.
