Fumigation Shut Off
Here's where to make requests for the discontinuance of gas service for the purpose of fumigation services. Appointment dates/times should be requested a minimum of 72 hours in advance. All requests to shut off gas service will be scheduled as morning appointments unless otherwise specified. Provide the type of fumigant being utilized, the company performing the work and the contact name and number of the representative requesting the service on behalf of the customer.
Fumigation Turn On
You can also use this form to make requests to restore gas service upon completion of fumigation services. Appointment dates should be scheduled when it is safe to re-occupy the structure. Four-hour appointment time frames will be accommodated only if resources are available. If resources are unavailable on the date/time requested, appointments will be scheduled as an all day 8 a.m. to 8 p.m. appointment. A "Notice of Re-entry" must be posted at the structure at the time personnel arrive to perform the service. Access to the premises and appliances must be provided in order to complete the requests. Provide the type of fumigant being utilized.
