Multiple Service Requests
Using Online Forms
Online forms are provided here for the convenience of property managers, developers and fumigators. Please use these forms to submit service requests for multiple transactions that can be scheduled to commence at least two business days from today, excluding weekends and holidays.
- If immediate service is required within 48 hours, please call our 24-hour Customer Service line at 1 (800) 743-5000. Please do not use these forms for immediate service needs. Thank you.
- If your service has been disconnected due to non-payment, please do not use the online form to reconnect your service. Please call our Customer Service line at 1 (800) 743-5000.
E-mail requests will be processed within one business day of receipt. Business days are Monday through Friday, excluding holidays. A confirmation response will be sent to you by e-mail. Please verify that your e-mail address and daytime phone number are correct in the event that we must contact you regarding your request.
Request Forms
Fumigation—To turn off or start gas service to conduct fumigation services, use this form.
Property Managers—For multiple transaction requests of the same type, please use these forms:
- Start Service (for which you will be the customer of record)
- Stop Service (for which you are authorized)
- Transfer Service (for which you will be the customer of record)
- Change Mailing Address (for which you are authorized)
Developers—For multiple transaction requests of the same type, please use these forms:
- Meter Set (Developers) (for which you will be the customer of record)
- Stop Service (Developers) (for which you will be the customer of record)
